Attach an additional sheet with the description of the problem and submit to your instructor for signature. The instructor must attach a statement indicating the process used for evaluation of the student's achievement and assignment of grade. Drop completed form off to the Student Affairs Coordinator at 119 Plant BioScience Building or email completed form to them; they will route the document for signatures and send a copy to the Registrar's office.  Be sure to indicate the number of HORT 492, AGSCR or BIOB 492 credits for which you would like to be enrolled.

HORT 498 Internship
Guidelines and Procedures for Internships
It is best to meet with your advisor first if you are interested in an internship. Follow instructions in the internship packet and attach a written proposal of the activities to be performed for credit and submit to instructor and employer for signature. Drop completed form off to the Student Affairs Coordinator at 119 Plant BioScience Building or email completed form to them; they will route the document for signatures and send a copy to the Registrar's office. Your advisor will help you determine the number of 498 credits for which you will be enrolled and in which semester.
